Agreement

By engaging with Asina Disability Support for disability support services, you agree to these terms and conditions. Our services are provided in accordance with the NDIS Practice Standards and the NDIS Code of Conduct.

Our Services

We provide NDIS-funded disability support services including Community Access, Supported Independent Living, Daily Living Support, Transport Assistance, Meal Preparation, Capacity Building, and Drop-in Support. All services are delivered in line with your NDIS plan and individual service agreement.

Service Agreements

Before commencing services, we will enter into a formal Service Agreement that outlines the specific supports to be provided, the schedule and frequency of services, pricing in accordance with the NDIS Price Guide, cancellation terms, and the rights and responsibilities of both parties.

Cancellation Policy

We require reasonable notice for cancellation of scheduled supports in line with NDIS terms. Short-notice cancellations (less than 2 business days) may be charged in accordance with the NDIS Price Guide.

Your Rights

As a participant, you have the right to be treated with dignity and respect, to receive services that are safe and of high quality, to have your privacy and confidentiality maintained, to provide feedback and make complaints, and to change or cancel your services at any time.

Our Responsibilities

We are committed to delivering services in line with NDIS Practice Standards, maintaining appropriate insurance and worker screening, providing properly trained and supervised support workers, keeping accurate records and reporting, and continuously improving our services based on feedback.
